We offer formal assessments for ages 13+:
- Drug & Alcohol Dependency and Addiction
- Sex Addiction and Problematic Sexual Behavior
- Personality Disorders
Drug and Alcohol Assessments:
30 minute collaborative call with current provider after initial meeting (optional)
75 minute interview session in person or virtual (in person preferred)
50 minute follow up session (in person preferred)
Assessments Administered Before Session
- DAST 10: Drug Abuse Screen Test
- Patient Health Questionnaire-9
- Alcohol Screening Questionairre
- TAPS 1 & 2: The Tobacco, Alcohol, Prescription Medication, and Other Substance Use Tool
- SASSI: Substance Abuse Subtle Screening Inventories (Adult and Adolescent)
A summary of assessment results, diagnostic impressions, treatment recommendations, and suggested resources (provided by print and/or pdf via secure email). A copy of results will be provided in session and reviewed with your therapist.
Recommendations for levels of care will be discussed in session. Our goal is to provide an objective perspective, to give insight, and to provide recommendations that match the clients needs. We collaborate regularly with various levels of care (IOP, PHP, Residential Treatment, Inpatient Care), so that we can make effective referrals if needed.
Pricing: Varies by therapist, but ranges from $900-$1500
Sex Addiction Assessment
